| Circle Center Adult Day Services | Adult day care program which enables older adults with special needs to continue living at home, and supports their family caregivers who work or need respite from 24-hour care. A multi-disciplinary team of geriatric professionals provides an individualized program of health, social and emotional services. Emphasis is on maintaining maximum wellness and independence through a structured program of nursing and personal care, therapeutic and enriching activities, friendships and emotional support and careful supervision up to 10 hours a day, six days a week. |

| A Grace Place Adult Care Center | Supports community-based long term care through adult day care services. Individuals with chronic and age-related health conditions receive essential health and social services compatible with personal wants and needs. Their families or primary caregivers receive respite from the demands of full-time care. Each service offered promotes the highest level of independence and health possible and can include nursing, health management, counseling, planned recreation, rehabilitation therapies and life skill training. |

As a nonprofit, Senior Connections accepts donations of items, services, monetary gifts and memorials. |
